MEDICAL TOURISM BENEFIT

Associates and dependents who participate in Cognizant’s medical plans are eligible to take advantage of the Medical Tourism Benefit. The benefit allows covered associates and dependents to receive 100% coverage for select, pre-approved surgical procedures when they elect to have a procedure performed at a facility in the Health Center Plus network. The facilities selected for Cognizant are located close to the US mainland. There is no deductible*, copay, coinsurance or other out-of- pocket expense. In addition, travel, lodging and a daily allowance will be provided for the patient and a family member, caregiver, or companion. The associate may also be eligible for paid time off and a payment of $2,000. In addition to certain other eligibility criteria, the surgery must be pre-approved and scheduled through the Health Centers Plus program at its designated facilities prior to receiving treatment. Payment is subject to otherwise applicable limits.

    Program benefits may include:
